Vegan and Vegetarian Diets and Canker Sores: The B12 Connection

Plant-based diets cut off the primary dietary source of B12 — and B12 deficiency is one of the most consistent nutritional drivers of canker sores. Here's what vegans and vegetarians need to know, and what to do about it.

Vegan and vegetarian diets don't cause canker sores directly — but they reliably increase the risk of deficiencies in B12, iron, zinc, and sometimes folate, all of which are independently associated with recurrent aphthous stomatitis (RAS). B12 is the most critical: it's found almost exclusively in animal products, dietary intake drops to near-zero on a strict vegan diet, and B12 deficiency is one of the strongest and most correctable nutritional drivers of canker sores. Vegetarians are less affected than vegans because eggs and dairy provide B12, but are still at elevated risk for zinc and iron shortfalls. If you switched to a plant-based diet and your canker sores got worse, deficiency is the most likely explanation — and it's testable and fixable.


Why Plant-Based Diets Create Canker Sore Risk

The connection isn't ideology — it's biochemistry. Canker sores in nutritionally deficient patients follow a consistent pattern: when the deficiency is corrected, outbreak frequency drops. The nutrients most implicated in RAS — B12, iron, zinc, and folate — are all harder to obtain or absorb on plant-based diets.

This doesn't mean plant-based diets are inherently unhealthy. It means that eliminating animal products requires deliberate attention to specific micronutrients that animal foods supply efficiently and plant foods either lack entirely or supply in less bioavailable forms.


Vitamin B12: The Core Issue for Vegans

Why B12 Is Almost Impossible to Get from Plants

Vitamin B12 is synthesized by bacteria and archaea — not by plants or animals. Animals accumulate B12 by consuming bacteria directly or by eating other animals. Plants, growing in increasingly sterile agricultural soil, accumulate essentially none. Fermented foods like tempeh and nori contain trace amounts, but not enough to meet daily requirements reliably, and the bioavailability varies.

The result: a strict vegan diet provides near-zero dietary B12. The only reliable solutions are supplementation or fortified foods.

How B12 Deficiency Causes Canker Sores

B12's role in RAS operates through two pathways:

Epithelial cell turnover: B12 is required for DNA synthesis via its role as a cofactor for methionine synthase. The oral mucosal epithelium turns over every 7–14 days — one of the fastest renewal rates in the body. Cells that can't replicate normally produce abnormal, fragile epithelium that is more susceptible to ulceration. Deficiency effectively weakens the mucosal barrier from below.

Immune regulation: B12 is involved in the methylation reactions that regulate gene expression in immune cells, including the T-cell populations involved in aphthous ulcer pathogenesis. Low B12 appears to shift the local immune environment toward greater reactivity — increasing the likelihood that the oral mucosa triggers the CD8+ T-cell attack that produces an aphthous ulcer.

The clinical evidence is substantial. Patients with RAS show significantly higher rates of B12 deficiency than matched controls. A well-known Israeli RCT (Volkov et al., 2009 — PMID: 19671390) found that sublingual B12 supplementation at 1000mcg nightly reduced canker sore frequency, duration, and pain in RAS patients regardless of their baseline B12 levels — suggesting B12 plays a role beyond simply correcting frank deficiency.

The Lag Problem: You Won't Feel It Right Away

B12 is stored in the liver — typically a 2–5 year supply. This is why vegans who don't supplement often don't notice symptoms for years after stopping animal product consumption. The stores deplete gradually. By the time canker sores appear or worsen, serum B12 may still look normal while tissue-level deficiency is already established.

This makes serum B12 an imperfect marker. A more sensitive measure is methylmalonic acid (MMA) — it rises before serum B12 drops, indicating functional B12 deficiency at the cellular level. If you've been vegan for more than two years without supplementing and are experiencing RAS, it's worth asking your doctor for an MMA test alongside standard B12.

What Form to Take

Not all B12 supplements are equivalent for oral mucosal health:

Methylcobalamin is the active neurological form and the one most relevant to the RAS studies. It doesn't require conversion in the body. Most RAS research uses this form or sublingual delivery.

Cyanocobalamin is the most common supplement form — cheaper and stable, but must be converted to methylcobalamin or adenosylcobalamin by the body. Adequate for most people but may be less effective for those with MTHFR variants that impair methylation.

Sublingual delivery bypasses the need for intrinsic factor (the protein required for gut absorption of B12), which matters for people with gastric issues. Most RAS-specific research uses sublingual dosing.

The dose used in the Volkov et al. RCT was 1000mcg sublingual nightly. Standard maintenance doses (250–500mcg daily) are probably sufficient for deficiency prevention; therapeutic doses for RAS reduction are higher.

Iron: The Issue for Vegetarians and Heavy Menstruators

Heme vs. Non-Heme Iron

Dietary iron exists in two forms: heme iron (from animal flesh) and non-heme iron (from plants, dairy, eggs). The absorption rate is fundamentally different:

  • Heme iron absorption: 15–35%
  • Non-heme iron absorption: 2–20%, and heavily influenced by dietary context

Phytates (in whole grains and legumes), polyphenols (in tea, coffee, wine), and calcium all inhibit non-heme iron absorption. Vitamin C consumed in the same meal enhances it. A vegan or vegetarian who eats iron-rich plant foods but drinks tea with meals and doesn't pair iron sources with vitamin C may absorb far less iron than their food diary suggests.

Iron Deficiency and RAS

Iron deficiency — specifically low ferritin (stored iron) — is consistently elevated in RAS populations. The mechanism parallels B12: iron is required for epithelial cell proliferation and for normal immune function. Deficiency impairs mucosal repair and alters the T-cell balance in the oral mucosa.

Vegetarians have lower iron stores on average than omnivores. Vegans are not consistently lower than vegetarians (legumes and greens are good plant iron sources), but both groups are at higher risk than omnivores due to the non-heme absorption disadvantage.

The combination of vegetarian diet + heavy menstrual bleeding is particularly high-risk — monthly blood loss depletes iron faster than plant-based diets typically replenish it. What to Test and What Levels Matter

Standard serum iron is a poor marker for RAS risk — it fluctuates widely day-to-day. Ferritin is the relevant measure. Ferritin below 30 ng/mL is associated with RAS in studies, even if it's technically within many labs' reference ranges. Some RAS patients respond to iron supplementation only when ferritin is brought above 50–70 ng/mL.

If your ferritin is low-normal (30–50), that's worth treating in the context of recurrent canker sores.


Zinc: The Overlooked Shortfall

Zinc is underappreciated in the RAS-nutrition connection. It plays a direct role in:

  • Epithelial repair and wound healing (zinc is required for keratinocyte proliferation)
  • Immune modulation (zinc-dependent enzymes regulate T-cell function)
  • Mucosal barrier integrity

Why Plant-Based Diets Fall Short

Phytates — abundant in legumes, whole grains, nuts, and seeds — are the primary dietary inhibitors of zinc absorption. These foods are staples of vegan and vegetarian diets. The result: even when zinc intake looks adequate on paper, bioavailability can be significantly lower than in omnivores.

The Institute of Medicine recommends vegans aim for 50% higher zinc intake than the standard RDA specifically because of this absorption issue. Most vegans don't know this and don't compensate.

Low zinc levels are found at higher rates in RAS patients than controls (Orbak et al., 2003 — PMID: 12657036). Supplementation studies show mixed results, but zinc appears most effective when actual deficiency exists rather than as a general supplement.

Folate: Usually Fine, But Worth Knowing

Folate is one area where plant-based diets generally do well — leafy greens, legumes, and fortified grains are rich sources. Most vegans and vegetarians are not at elevated folate risk.

The exception: people who are pregnant, taking methotrexate or other folate antagonists, or who have MTHFR gene variants that impair folate metabolism. In these cases, supplementation with methylfolate (the active form) rather than folic acid is preferable.

For most plant-based eaters, folate is not the primary concern. It's worth including in a deficiency workup if B12, iron, and zinc come back normal and RAS is still frequent.


Vegetarians vs. Vegans: The Risk Is Not Equal

Vegetarians who consume eggs and dairy have meaningful B12 protection:

  • One large egg provides roughly 0.5mcg B12 (daily requirement is ~2.4mcg)
  • A cup of milk provides ~1mcg B12
  • Cheese and yogurt are also reliable sources

A lacto-ovo vegetarian who eats eggs and dairy regularly is unlikely to become B12-deficient from diet alone. Their canker sore risk through the B12 pathway is substantially lower than a strict vegan's.

However, vegetarians are not risk-free:

  • Iron: The heme/non-heme absorption gap still applies. No meat means lower iron absorption efficiency.
  • Zinc: Phytate inhibition affects vegetarians who eat legume-heavy diets just as much as vegans.
  • Omega-3s: Not a direct RAS factor, but anti-inflammatory DHA/EPA from fish may play an indirect role in mucosal health. Vegetarians who eat no fish may want to consider algae-sourced DHA.

Practical Steps If You're Plant-Based with Frequent Canker Sores

Step 1: Get Tested

Before supplementing, get actual numbers. The relevant panel:

  • Serum B12 and methylmalonic acid (MMA) — MMA is more sensitive for functional deficiency
  • Ferritin — not serum iron, not hemoglobin; ferritin specifically
  • Serum zinc — less reliable than B12/ferritin but worth including
  • RBC folate — more accurate than serum folate

Your GP can order all of these. Context matters: tell them you're vegan/vegetarian and experiencing frequent canker sores. Some doctors won't test MMA unless prompted.

Step 2: Supplement Strategically

If deficiency is confirmed:

B12 (vegans, required): 1000mcg methylcobalamin sublingual daily. Even if serum B12 looks adequate, MMA elevation indicates functional deficiency. This is non-negotiable for strict vegans — diet alone cannot supply enough.

Iron (if ferritin < 50): Iron bisglycinate is the gentlest form for supplementation (less GI side effects than ferrous sulfate). Taken with vitamin C, away from calcium and coffee.

Zinc (if deficient): Zinc picolinate or zinc bisglycinate are the most bioavailable forms. 15–25mg elemental zinc daily. Don't exceed 40mg long-term — high zinc depletes copper.

Step 3: Optimize Absorption from Food

Even without supplementation, some dietary adjustments improve micronutrient absorption from plant foods:

  • Soak and sprout legumes and grains — reduces phytate content and improves iron and zinc absorption
  • Pair iron-rich foods with vitamin C — a squeeze of lemon on lentils meaningfully improves iron uptake
  • Avoid tea and coffee with meals — polyphenols inhibit non-heme iron absorption; wait 1 hour after eating
  • Ferment when possible — fermented soy (tempeh, miso) has lower phytate content than unfermented

Did Your Canker Sores Get Worse After Going Plant-Based?

This is a common pattern and almost certainly nutritional. The B12 stores that protected you during the transition eventually deplete — typically 2–5 years after stopping animal products without supplementing. The appearance (or worsening) of canker sores in a long-term vegan is a recognized clinical signal of B12 depletion.

The fix is straightforward: supplement, get tested to confirm correction, and give it 3–6 months to see improvement in outbreak frequency. The mucosal epithelium needs time to normalize once deficiency is corrected — results aren't immediate.

If you recently went plant-based and canker sores appeared within months, B12 is less likely the cause (stores take longer to deplete) — look at zinc and iron first, since those can drop faster.
